After a major restructuring of New Jersey's mafia family, the FBI recognizes mobster Tony Soprano as Jersey's sole mob boss with Uncle Junior in prison facing racketeering charges. Big Pussy Bompensiero reappears, asking for Tony's approval to return to the family. The boss accepts Big Pussy but still questions his loyalty. The reunions continue when Tony's commune-living sister, Janice, unexpectedly arrives from Seattle, Washington. Under Carmela's insistence, Tony attempts to continue his therapy. Rejected by a new therapist, Tony tries to convince Dr. Jennifer Melfi to counsel him again. Dr. Melfi, who has been in hiding and seeing patients from a motel room, angrily refuses to help.
